[specimen] GUI: new look (2nd try)

Hi.

I've had few spare moments recently, but I've managed to contribute some of 
them to Specimen. I've focused on the per-channel volume fix as previously 
discussed.

Simply disabling the MIDI controller for individual patch volumes isn't good 
enough, we need to make it adjust a separate channel volume setting instead. 
And since we don't have that, it needs to be written. I already have some GUI 
modifications ready which I'd now like to present for review.

Notice that I've added a channel panning setting as well because panning 
behaviour also suffers from the same root problem (i.e. all patches' panning 
is modified by a single MIDI controller). Another thing worthy of attention  
is that the keyboard is in the bottom now because I think it fits there much 
better.

The attached image is not a mashup, the code's ready but it's not in SVN yet.
